Ashwagandha Side Effects, Interactions, and Who Should Skip It

Ashwagandha (Withania somnifera) is one of the most searched adaptogen supplements—and for good reason. Short-term studies suggest some extracts may help with feelings of stress and, in some trials, sleep quality. That interest also means more people are asking the harder question: who should skip it, what side effects show up, and which medicines might interact?
This guide summarizes what public U.S. health agencies currently say. It is not a green light to start a bottle—or a scare piece. The goal is clearer expectations before you buy or keep using ashwagandha. When marketing language drifts toward “cures stress” or “fixes hormones,” treat that as a signal to slow down and re-read primary sources.
What agencies say about short-term use
According to the NIH Office of Dietary Supplements (ODS) consumer fact sheet on ashwagandha, extracts used in studies appear to be reasonably tolerated for up to about three months in many adults. The same sheet notes that long-term safety is not well established. The National Center for Complementary and Integrative Health (NCCIH) echoes that short-term use may be safe for many people, while long-term conclusions are still limited.
In trials, people most often report mild issues such as stomach upset, diarrhea, nausea, or drowsiness. Those symptoms are usually the reason someone stops—not dramatic reactions. Still, “usually mild” is not the same as “risk-free,” and rare events matter precisely because they are easy to dismiss until they happen to you or someone you care for.
ODS also emphasizes that study products were not identical. Root-only extracts, root-and-leaf extracts, and whole-root powders all appear in the literature. That heterogeneity means a side-effect profile from one product does not automatically transfer to another—and that “ashwagandha” on a label is a category, not a single standardized drug.
Side effects people actually report
- Digestive discomfort — nausea, loose stools, or stomach upset are among the more common complaints in research and consumer reports. Taking capsules with food sometimes helps; it does not guarantee comfort.
- Sleepiness or sedation — some people feel calmer; others feel too drowsy, especially if they also take sedating medicines, alcohol, or nighttime antihistamines.

- Autoimmune or thyroid disorders — NCCIH notes it is not recommended for people with these conditions without clinician oversight. Self-experimentation is a poor substitute for lab monitoring when thyroid medicine is already in play.
- Upcoming surgery — stop and ask your surgical team; sedative and blood-pressure effects matter perioperatively.
- Known liver disease or unexplained enzyme elevations — given rare hepatotoxicity reports, many clinicians prefer alternatives or close monitoring.
- Children and adolescents — most adult trial data do not automatically extend to pediatric use; ask a pediatric clinician rather than dosing from an adult bottle.
Medication interactions to ask about
NCCIH lists possible interactions with medicines for diabetes and high blood pressure, immunosuppressants, sedatives, anticonvulsants, and thyroid hormone medicines. Separating study context from personal dosing
ODS describes trial doses that often land around a few hundred milligrams of extract daily, with some sleep-related signals looking clearer near higher studied extract amounts. Those figures are research context. They are not a instruction to chase the maximum number on every label, and they assume the extract resembles what was studied—not a gummy with undisclosed standardization.
If a product cannot tell you plant part, extract strength, or testing, you cannot meaningfully map it onto agency summaries. In that case, the safest “dose” is often zero until you find a clearer bottle.

Ashwagandha is studied for stress-related outcomes and sleep in short trials, and many adults tolerate it for a few months.
